Your First Visit
At your child’s first visit at St. Apollonia Dental Clinic, you and your child will be introduced to our dental team and to the dental office in general. We will review your child’s medical history. Please bring your insurance card and our New Patient Packet already completed. If you do not bring the packet with you, please come 15 minutes prior to your scheduled appointment to allow time for completion of the necessary paperwork. A parent or legal guardian must attend every appointment with the child/children if they are under age 18. It is recommended that a child has their first visit by his/her first birthday.
We look forward to meeting your child!
